MENTALLY PHILL is self-care gamified.

Picture your depression as a sadistic troll and name it. Make a list of the ways it likes to hurt you, then make a list of ways you can spite it right back.  Make that troll rue the day it decided to haunt you. 

Note: I specifically mention depression in this game, but it can be used for any mental illness that makes self-care tasks difficult for you. 

This game is the size of a business card, so feel free to print it out and write your lists on the back. Put the card in your wallet for when you need it. This game is free to download, but please consider donating a dollar or two if you find it helps you.
